COOKING WORKSHOP REGISTRATION, WAIVER & BEHAVIOUR AGREEMENT
This Enrolment and Acknowledgement of Risk Form ("Agreement") is entered into by the parent/legal guardian of the child participating in the Cooking Workshop facilitated by Authentic Learning Australia ("ALA").
MEDICAL & DIETARY INFORMATION
In the registration eform below, you must list any known medical conditions, food allergies, or dietary restrictions (including, but not limited to nuts, dairy, gluten, eggs, etc.). Please describe any signs, symptoms or management plans we should be aware of in the event of an allergic reaction or medical episode. Please provide us with a copy of your medical plan by attaching it to the booking/enrolment eform below.
By submitting the registration eform below, you acknowledge and agree that:
It is your responsibility to inform ALA in writing of any allergies, dietary restrictions or medical
conditions before your child participates in the workshop.
While all reasonable care will be taken, ALA cannot guarantee an allergen-free environment.
You release ALA and its staff from any liability arising from the consumption of food that may trigger
an undisclosed allergy or reaction.
RISK ACKNOWLEDGEMENT
You understand that your child will participate in activities involving, but not limited to:
The use of sharp tools (e.g. knives, graters, peelers)
Hot equipment (e.g. stovetops, frying pans, ovens)
Handling of raw and cooked foods
ALA will take all reasonable steps to ensure the safety of all participants, however:
You acknowledge that there are inherent risks associated with cooking and food preparation.
You agree that you will not hold ALA, its staff, volunteers, or affiliates liable for any injury, accident,
illness or damage that may occur during the workshop, except in the case of proven gross
negligence.
FOOD TAKEN HOME
Participants may take home a portion of food prepared during the session.
You understand this food has been prepared in a shared kitchen environment under supervision but
may contain allergens or ingredients that require refrigeration or reheating.
You accept full responsibility for any consumption of food taken home.
BEHAVIOURAL EXPECTATIONS
Authentic Learning Australia is committed to maintaining a safe, respectful and inclusive learning environment. To ensure the wellbeing of all:
Participants are expected to follow instructions, show respect to team members and peers, and use
kitchen tools responsibly.
Disruptive, unsafe, or aggressive behaviour will not be tolerated.
ALA reserves the right to withdraw any participant from the program without refund should their
behaviour place themselves or others at risk, or consistently disrupt the group.
DROP-OFF, SUPERVISION AND SIGN-OUT
This is a drop-and-go program. Children will remain under the supervision of qualified ALA staff for
the duration of the workshop.
Sign-in and sign-out is required. Parents or authorised guardians must sign their child in at the
beginning of the session and sign them out upon collection.
Children will only be released to the parent/guardian listed on this form or those authorised in
writing.

You agree to all terms and conditions outlined above and within the booking/enrolment eform below
You release Authentic Learning Australia and its staff from any liability arising from participation,
